Varicose Vein Surgery

An Introduction

No need to Cover Your Legs Anymore!

The Surgery often requires a short hospital stay, usually no more than one night. The time of admission is usually a few hours before the operation and this allows some simple preparations to be made. Surgery is usually performed under general anaesthesia, but a local anesthesia with sedation is an alternative for patients who would prefer to be awake during surgery. You should be able to go home the day after surgery. Before you go home, the bandages will be removed and firm compression stockings/garment applied which should be worn for two weeks.

What are Varicose Veins

The heart pumps the blood to all body parts through arteries, so arteries bring blood from the heart to the extremities and the Veins are blood vessels that returns blood from your body’s tissues (like extremities) towards your heart. Veins contain one way valves that allows the blood to flow in upwards direction only by the movement of limb muscles, so that the blood can easily returned back to the heart.

When there is obstruction of veins due to any reason, or when prolonged distension of the veins due to blood stasis, the valves stretch and no longer effective. This allows blood to travel back down the veins towards the feet. The veins in the legs that are near to the surface of the skin enlarges & become tortuous and result in what is commonly called varicose veins. These can range from minor dilatations to large bunch of grape-like structures in the calf. Varicose veins literally mean grossly dilated, tortuous knotted veins. It can occur at different places in the body but lower limb (especially the calf & thigh) is most affected. Varicose veins are not usually a serious health problem, but they can be painful and you may be bothered by the way they look. Varicose veins is a slowly progressive disease which if left untreated can lead to marked skin change damage or ulceration near the ankle. In the early phases treatment is given for cosmetic reasons, but later it becomes very distressing by discoloration, edema of legs, Eczema, recurrent ulcerations around the foot and ankle joints. Finally it produces painful non healing ulcers in the foot and ankle region.

Sometimes there are smaller thin veins are noticed in your extremities or any other parts of body, this condition is known as Spider Veins. This may require treatment for aesthetic reason.

Are you a good candidate for Varicose Veins?

Varicose veins may cause feelings of fatigue, heaviness, aching, burning, throbbing, itching and cramps in the legs. These symptoms are often accompanied by swelling of the ankle, which frequently appears after long hours of standing. Some people are very troubled by the aching that varicose veins produce. If you have such symptoms, you may be a good candidate for the procedure.

The Procedure

The procedure of varicose vein depends on its location, its severity, amount of tortuosity and the status of valve & perforators.

If there is only dilation & tortuosity of veins but the valves & perforaters are intact, you may get benefited by Sclerotherapy. Sclerotherapy is injection of some special drugs called sclerosant in side the veins in its empty condition and rapidly giving a compression over it by compression bandage or garments. These drugs cause shrinkage & obliteration of veins in due course of time. You may need many sittings of sclerotherapy depending on the extent & severity of varicose veins.

If the varicose veins are associated with incompetent/damages valves & incompetent perforators (identified by Colour Doppler examination), you will also require surgery for ligation of these incompetent perforators & valves along with sclerotherapy for a successful treatment.

Your Recovery

Following surgical treatment for varicose veins, it is best to sit with the feet elevated and to avoid standing still for extended periods. The immediate effects of the operation includes some discomfort and bruising of the legs. The discomfort is usually not severe and can be treated with simple analgesics. The bruising may last 2-4 weeks. You need to wear compression garments for long time after the sclerotherapy or perforators ligation.

Expected Results

The long term outcome of carefully performed varicose veins surgery is excellent. The scars fade to tiny white marks over a period of several months and become very difficult to see. Although treatment can help existing varicose veins, it can’t keep new varicose veins from forming. So you have to follow the doctors advise regarding compression garment, limb elevation for a long time to avoid recurrence or development of newer varicosities.
